- Additional gingivitis cleanings available for $120 per visit
- 2 Routine Dental Exams
- All Necessary X-Rays (bitewings, PA’s, FMX as needed)
- 2 Fluoride Treatments
- 1 Emergency Exam (if needed during the plan year)
- Up to 20% OFF Additional Treatments, including:
- Fillings
- Crowns & Bridges

- Root Canals
- Periodontal Therapy

Terms & Conditions
- The annual membership fee is non-refundable and non-transferable.
- Plan is valid for 12 months from the date of enrollment.
- Discounts cannot be combined with dental insurance or other promotions.
- Included cleanings apply only to patients without periodontal disease.
- Patients with periodontal disease may apply the 20% discount toward periodontal therapy.
- Payment for treatment outside the plan benefits is due at time of service.
- This plan is *not* dental insurance; it is an in-office savings option administered solely by Dehyar Dental.
